It seems pretty simple. Piece together the rod sections and insert into the oval opening between the bumper and tailgate. But it doesn’t connect to anything in there. What am I missing here?

There is a metal tube inside that plastic hole in the bumper that guides the rod. You should feel it engage and give resistance as you crank down. Are you getting missing that guide?

No matter how many times I wrote this, I could not make it sound any less dirty.....

Can't find the hole inside the hole. Can't even imagine trying to get that spare off by the side of the road at night.

There is nothing that automatically guides the rod into the lowering mechanism. You have to have the rod going in fairly straight, then the white plastic 'cone' guides it in.

Lowering spare starts at about 1:07:

The extension pieces go in level, but you have to follow the angle (right to left) that the guide hole is set on.

If you start just the pieces through the hole, lay down and look up at behind the rear bumper and you can see the white funnel and guide it in by hand. After you do it a couple of times it becomes second nature.

If you look up under the truck it's very straight forward.

Or if you put your cell phone flashlight into the hole you see the alignment of the white receiver and you can tell where to angle it.

Not gonna lie, my first attempt at it brought back memories from nature documentaries, wondering how the act is completed without a guiding hand.....

The square drive that the tool goes into is deep in there. Just make sure you use the end with the wider opening.
