Yes part numbers are the same 68251037AC Not sure about the harness, but you'll need something like a tazer JL to activate the sway bar disconnect EVO also has a manual and an on board air solution to activate the disconnect: Manual - EVO-1087 air - EVO-1152

Skid only has the front brackets, will need the back brackets

Which brackets are you referring to? The ones on the metal skid that bolts onto the frame? If so, it does and it comes with the hardware too. Does not have the license plate holder, but you can drill one on.
