This pretty much only happens with water that is pretty clean, not often with tap water.

How detailed do you want? The top freezes first, then the spike forms because as water freezes it expands.

So long as you have liquid water it will grow.

I have found that our tap water is pretty clean so if I set the freezer low they will form.

Yes. And it freezes edges first so it has to expand up through that final hole, often forming an ice tube.
