Thanks for all the comments. I’m going to take it to a discount tire and get the professionally done plug/patch combo and hope for the best.

Forgot to update... the tech at Discount Tire's brother is a policeman. He said it looked an awful lot like the spikes they use for disabling vehicles. If you look closely, these are not screws, they are definitely spikes of some kind. He said they are designed to break off the main assembly when they get run over and eventually get flung out of the tire, which opens the puncture holes they created.Must be contagious...
