I had a Friend who received a incorrectly milled Cam that was a few degrees off had to send back and they gave him another one. Only one I ever heard of .
I have seen it happen, but it is very rare. The tone wheel was in perfect time with the phaser, so it didn't cause any DTCs, just very poor performance. Careful inspection revealed that it was ground out of time, it was far enough to be obvious (looked to be about 15 degrees just by eye), but I didn't bother with putting a degree wheel and dial indicator on it to find out exactly how much.

If I am not mistaken the one He had after He sent it back they confirmed it to be 8° out of time.
Like I said its the only one I ever heard of . Has to be super rare as you said.
Oh and the alignment tool would not quite fit in the alignment hole in the cam. His was throwing a code. Switched phaser first to see if it was that ..nope still thrower code same side thats how he figured it out that it truly was the cam. Think is what He told me . 😂
